Referral Form
As a Registered Midwife Lactation Consultant, my services are covered by MSP from pregnancy until 6 weeks postpartum. I accept referrals from Registered Midwives, Physicians, Nurse Practitioners, Registered Nurses, and Registered Social Workers.
I also provide private pay consultations for families over 6 weeks postpartum or uninsured, dealing with complex feeding difficulties, tongue ties, poor infant weight gain, and other concerns for clients. After 6 weeks postpartum, clients do not need a referral and can contact me directly.
My services include:
Prenatal consultation (virtual or in-home)
will roll over into ongoing postpartum infant feeding and lactation support
Postpartum consultation (in hospital at VGH, at Jeneece Place, or in-home)
for a variety of infant feeding and lactation difficulties
for parents with a baby admitted to NICU or Pediatrics for higher level care for both infant feeding support and short-term postpartum care
Full-scope postpartum care (in hospital at VGH, at Jeneece Place, or in-home)
for dyads not attached to a primary care provider, clients who could benefit from continuity of care with in-home postpartum visits from birth-6 weeks postpartum, and in cases of adoption or surrogacy in the first 6 weeks of life
